Why Your Travel Nurse W-2 Has Withholding for the Wrong State

By Sharon Ben-Moshe · October 3, 2026 · Updated October 17, 2026

When travel nurses receive their Form W-2 in January, discovering state tax withholding errors is surprisingly common. Box 15 may show state income taxes withheld for a state you never visited, or show zero withholding for the state where you completed a 13-week assignment.

Staffing agency payroll departments handle thousands of traveling healthcare clinicians across all 50 states. Clerical mistakes, misconfigured staffing software, and confusion over reciprocity often lead to state tax reporting errors.

Why state withholding errors occur on travel W-2s

The most common reason for a state withholding mismatch is that an agency payroll clerk recorded your home state address as your work location, or recorded the agency corporate headquarters (such as Nebraska, Florida, or California) as the taxable work state.

Under federal and state tax law, state income tax must be withheld based on the physical location where the healthcare clinician performs patient care services. If you worked at a hospital in Ohio, your agency must report Ohio-source wages in Box 16 and Ohio withholding in Box 17, regardless of where the staffing agency is based.

Examining Box 15, 16, and 17 on Form W-2

Look closely at the bottom section of your Form W-2. Box 15 identifies the state abbreviation and the employer state tax identification number. Box 16 shows the taxable state wages, and Box 17 shows state income tax withheld.

If you worked assignments in three different states, your Form W-2 should contain separate state lines for each jurisdiction, with Box 16 wages adding up to your total taxable earnings.

If an agency erroneously assigned all your wages to a single state, you will appear delinquent in the states where you actually worked, triggering automated deficiency notices from state revenue departments.

How to resolve the error before filing

Do not simply alter numbers on your tax return without supporting documentation.

First, contact your agency payroll department and request a formal explanation and an amended wage statement, Form W-2c (Corrected Wage and Tax Statement).

Second, if the agency refuses to issue a corrected form before the filing deadline, you must file a resident return in your home state and a nonresident return in the assignment state. Attach a written statement explaining that Box 15 was misreported by the employer, along with copies of your travel contract, assignment confirmation, and pay stubs showing work facility addresses.

Frequently asked questions

Why did my staffing agency withhold income tax for the wrong state?

Payroll clerks frequently assign wages to the nurse home address or agency corporate headquarters rather than the state where patient care was delivered.

How do I read Box 15, 16, and 17 on Form W-2 for multi-state work?

Box 15 lists the state code, Box 16 shows state taxable wages, and Box 17 shows state tax withheld. Each state where you worked should appear on a distinct line.

How do I request a corrected Form W-2c from my staffing agency?

Contact your agency payroll department in writing, provide contract shift records, and request an amended Form W-2c showing proper state wage allocations.
